Getting Started
Request Service
- Please complete all fields and be as specific as possible. Each issue will be answered in order of date received.
- All changes must have the approval of the Chairperson or Director of the Program.
- When the form is complete, please submit the TDX Ticket. This form will be sent directly to the Student Data and Registrar and you will receive a notification it was submitted.
