Getting Started

 
 

Request Service

  1. Please complete all fields and be as specific as possible. Each issue will be answered in order of date received.
  2. All changes must have the approval of the Chairperson or Director of the Program.
  3. When the form is complete, please submit the TDX Ticket. This form will be sent directly to the Student Data and Registrar and you will receive a notification it was submitted.
